Register via Username
1. Click Create An Account in the black box on the right.
2. Create a username and password.
3. Enter your first and last name, date of birth, and zip code.
4. Enter the security code provided in your welcome email.
5. Your name must match the name you provided the practice when you scheduled your appointment.
6. Select security questions and provide answers.
7. Once logged in, click the Update Information button to fill out your mandatory medical history.
